Outlook 2003 Custom form

I've made a custom contacts form in Outlook 2003, and I want to email it to another computer. How can I save the form as a .oft file so I can email it? Please help - I've been tearing my hair out over this for a while.

Oooops. It's just typical that after slaving away at a problem for hours, I find the solution minutes after posting a thread about it. Typical... it was such an obvious solution as well...
