BBA ATi Radeon 9800 PRO 128MB samsung memory
512MB DDR
ASUS A7N8X 2.0 Deluxe

The attached picture should explain all. I have tryed fromating my HDD and the problem is still there. I have used many different driver version (Catalyst and nForce). The problem is happening a lot more frequently now.

your video card is dead. nforce drivers are not for ati cards. they're for the motherboard. anyhow, you need catalyst drivers, but right now it doesn't matter. that card is dying, on its way to dead. RMA it.

btw, i just read "it's happening a lot more frequently" and again your vid card is dying, on its way to being completely dead. although do check to make sure the heatsink is on firmly and making contact with chip.

With a dead video card, and a dying HD, I would tend to think that you have heat problems inside your case. You might want to dust the inside of your computer or invest in some case fans. If you don't address it, more things will die.
