Omegasearch just won't go away.

I've tried your omegakiller many times. even in safe mode. but it just can't get rid of it. I still have the small toolbar at the top of my explorer window. Here is my printout from hijack this. Does anyone see anything that i should get rid of?? (Omegakiller gets rid of the bin mapi one every time i run it but it just keeps coming back.)
