How do I record computer application live with narration?

Can anyone give me some information on software that will record my computer display live? I would like to make a .avi of my computer display and narrate what I am doing in an application. Like video tutorials that are available from Total Training, Lynda.com etc. Does it require an external video device such as camcorder?

    matlab could do it. but then again, matlab can do pretty much everything
