Defragmentation

Every so often, i defrag my computer. Recently, I came up with a problem that whenever I defrag my computer, it gets defragmented again right after restart. I defragment my computer (i wait for around 10-20 minutes) and I reboot and I check again and it is just as defragmented as before.

How do I fix this?

    How much is it defragmented upon rebooting? Can you post a screenshot of the color coded GUI and the defrag analysis.
