Urtbk infection, keeps coming back
My computer has some sort of infection that causes it to periodically -- about every 3 minutes or so -- open its own Internet Explorer windows (I haven't ever used Internet Explorer a single time on my own since I've gotten this computer). The window always directs immediately to a url.urtbk.com page.
If the IExplore program is open for longer than five seconds or so, it also starts ctfmon.exe (I keep my Task Manager open at all times and try to end the process immediately every time, as I'm not sure what kind of information this trojan could be getting from me if left open). I've also recently noticed that ctfmon.exe is in my list of programs to be opened upon startup, and despite having removed it from that list many times with CCleaner and msconfig, it keeps coming back. I've used Malwarebyte's Anti-Malware to relieve myself of this problem a bunch of times, but it always somehow comes back within a day. The infection usually comes back beginning with one or two instances of rundll32.exe being opened, and although I end them immediately, the iexplore popup barrage starts in less than a minute. I've also noticed that ctfmon.exe opens everytime I open Windows Media Player, and I don't know what to make of that. Here is my Hijackthis log:

The Windows Recovery Console will allow you to boot up into a special recovery (repair) mode. This allows us to more easily help you should your computer have a problem after an attempted removal of malware. It is a simple procedure that will only take a few moments of your time.
Once installed, you should get a prompt that says:

Please continue as follows:
(1) Close/disable all anti virus and anti malware programs so they do not interfere with the running of ComboFix.
(2) Click Yes to allow ComboFix to continue scanning for malware.
When the tool is finished, it will produce a report for you.
Please include the MBAM log along with C:\ComboFix.txt for further review, so that we may continue cleansing the system.
Caution: Never run and remove files with Combofix unless supervised by a qualified security analyst who is experienced in the use of Combofix. Misuse can cause serious computer problems.
